Therapeutic Horticulture: What It Is and What It Is Not

A careful introduction to therapeutic horticulture, horticultural therapy, professional boundaries, and the current evidence base.

Two related terms

The American Horticultural Therapy Association distinguishes horticultural therapy from therapeutic horticulture. Horticultural therapy is a treatment modality delivered by a registered horticultural therapist within an established treatment, rehabilitation, or vocational plan. Therapeutic horticulture is broader: a trained facilitator uses plant-related activities to support stated program goals.

That distinction matters. A university seminar, employee workshop, or garden-based learning program should not be described as clinical treatment unless it is designed and delivered under the appropriate professional and institutional controls.

What the evidence can support

Reviews report promising associations between gardening participation and aspects of wellbeing, social connection, and quality of life. They also describe substantial variation in program design, participant groups, measures, and study quality.

The responsible conclusion is not that nature or gardening cures illness. It is that structured horticultural activity is a credible area for education, participation, and further research, with clinical claims requiring stronger evidence and qualified oversight.

Begin by defining whom the program serves, what participants will do, where it will happen, and what the organization can realistically support. Bring in registered or licensed specialists whenever goals become clinical, diagnostic, or treatment-related.
